Alternatively, if you prefer to submit your Claim Form by either postal mail or fax, you may obtain the Claim Form by clicking HERE and then follow the included instructions to return your Claim Form by postal mail or fax.

IMPORTANT: Screenshots or PDFs of this Online Claim Form will not be accepted by mail or fax. Therefore, if you would like to submit a Claim Form by either postal mail or fax, please obtain the Claim Form by clicking HERE, and then mail or fax the completed Claim Form (along with the necessary proof) by no later than June 8, 2018. Any Claim Form that is sent by postal mail must be postmarked with a date that is not later than June 8, 2018.
